COVID-19 Vaccines for Faculty and Staff Household Members

Dear 91女神 staff and faculty,
Thanks to the generosity of the Missouri National Guard, we are excited to announce that we will soon be able to invitehousehold members (over age 18) of our full- and part-time faculty and staff to get vaccinated, too.
We will be offering the Moderna vaccine.
In order to identify eligible household members, we ask that you. The survey helps us narrow down who fits into which prioritization phase/tier according to.
What you need to know:
- Survey for employees: Full- and part-time 91女神 employees shouldto indicate their own eligibility and interest in being vaccinated.
- What about loved ones who are not household members? We hope to extend our invitations at a later date to include more members of the
community, but are limiting it to household members at this time.
- Banner/employee ID: The survey requires household members give the Banner/Employee ID for the associated
91女神 employee. That number can be found in Workday.
- Multiple household members: You can have multiple household members fill out the survey with the same associated
91女神 employee. They just each need their own separate entry.
- Age restriction: Please note that the Moderna vaccine is only for those 18 and older. Please do not
register minors at this time.
- Does it matter if you live in Illinois or Missouri? No. As long as you鈥檙e employed by 91女神, we can vaccinate you and your eligible household
members.
- What about the second dose? We will schedule everyone鈥檚 second dose after they get their first vaccine.
- Next steps: We will share registration information to household members based on phase/tier eligibility under.
We have several clinics scheduled for this week, so please fill out the form, or have your household members fill out the form,as soon as possible.
Reminder: We willnottell you which phase/tier you or your loved ones fit into. Please review the guidance prior to making that discernment. Some of the details from the state are open-ended and we rely on you to determine where you fit based on your personal circumstances.
